On 28 February 2018 the Disciplinary Tribunal found Ms Colleen T Yu breached:

- Article 39(a)(ii)B.2) of the Constitution in that she was guilty of conduct derogatory to, or not in the best interests of the Company or its Members,
in lodging client documents with the ATO which she had falsified without the knowledge of the client or her employer, then destroying all records following an ATO investigation.
Ms Yu attended the Disciplinary Tribunal Hearing and admitted the Complaints.
Penalty and costs
The Disciplinary Tribunal found the Complaints sustained and imposed the following penalties:
- forfeiture of Membership
- Ms Yu is not eligible for readmission to Membership until 28 February 2026
- prior to any application for readmission to Membership, Ms Yu must first complete the Ethics & Governance segment of the CPA Program (or its equivalent).
Ms Yu was ordered to pay CPA Australia’s costs of $655.56 prior to any application for readmission.
