On 30 April 2015 the Disciplinary Tribunal found Ms Meredith R Savins breached:

• By-Law 9.5(b) in failing to successfully complete a Quality Review (QR) by the due date of 20 June 2014
• Article 39(a)(ii)I of the Constitution in failing to respond to three letters, emails and eight telephone messages from Professional Conduct in relation to a Complaint made about her regarding her QR.

Ms Savins did not attend the Disciplinary Tribunal Hearing.

Penalty and costs 

The Disciplinary Tribunal found the Complaints sustained and imposed the following penalties:

• in relation to her QR, Ms Savins is to successfully complete a compliant QR by 31 May 2015, a failure to so comply to result in forfeiture of Membership until she has done so
• Ms Savins is to provide proof that she holds a current policy of professional indemnity insurance to $2 million by 15 July 2015, a failure to so comply to result in forfeiture of Membership until she has done so
• in relation to her failure to respond, a severe reprimand and a fine of $1000.

Ms Savins was also ordered to pay CPA Australia’s costs of $812.65.

Ms Savins failed to complete the QR and her membership was forfeited.
