On 26 March 2015 the Disciplinary Tribunal found Mrs Carolyne G Dunsford breached:

• By-Law 9.5(b) in failing to successfully complete a Quality Review (QR) despite being selected in 2011 and signing two agreements with CPA Australia that she would complete her QR by a certain date
• By-Law 5.2(a)(ii) in failing to respond to five letters and five telephone calls from Professional Conduct in relation to a Complaint made about her.

Penalty and costs 

The Disciplinary Tribunal imposed the following penalties:

• forfeiture of Membership until such time as Mrs Dunsford has successfully completed her QR
• prior to any application for readmission, Mrs Dunsford must first pay CPA Australia the costs of her QR
• in relation to her failure to respond, a severe reprimand.

Mrs Dunsford was also ordered to pay CPA Australia’s costs of $944.87 prior to any application for readmission.
