On 29 April 2014 the Disciplinary Tribunal found Mr Vincent M Cook was ‘Closely Associated with a Practice Entity which has become Insolvent’, contrary to Article 39(a)(iii) of the Constitution, in that as sole director of Vince Cook & Associates, on 10 April 2013 Mr Cook placed his practice into voluntary administration.

Mr Cook attended the Disciplinary Tribunal Hearing and admitted the Complaint.

Penalty and costs 

The Disciplinary Tribunal found the Complaint sustained and imposed the penalty of forfeiture of Membership until 10 April 2016.

The Disciplinary Tribunal also ordered that prior to any application for readmission to Membership, Mr Cook must:

• successfully complete the Ethics & Governance segment of the CPA Program (or its equivalent)
• successfully complete a Quality Review of his practice if he is a Principal in Public Practice.

Mr Cook was also ordered to pay costs of $782.16.
